Common options include topical treatments like creams, lotions, and ointments, and systemic treatments like oral medications and injections.

Psoriasis

 

Psoriasis is a chronic autoimmune skin disease that speeds up the growth cycle of skin cells. It causes the skin cells to multiply 10x faster than their normal rate. Bumpy red patches covered with white scales can appear on any area of the skin, but they are most commonly seen on the scalp, elbows, knees, and lower back. On darker skin, these patches may appear purplish with gray scales.

 

Types of Psoriasis

 

There are 6 types of psoriasis, namely:

 

  • Plaque psoriasis: The most common type of psoriasis, characterized by the accumulation of skin cells that result in scaly, itchy, and dry patches.
  • Pustular psoriasis: A condition that causes red and scaly skin, with small pusfilled bumps appearing on the palms of the hands and soles of the feet.
  • Inverse psoriasis: It causes bright red, shiny lesions to appear in skin folds, such as the armpits, groin, and under the breasts, and is known as inverse psoriasis.
  • Psoriatic arthritis: A combination of two conditions that lead to inflammation of the skin and joints, resulting in swelling, stiffness, and discomfort.
  • Guttate psoriasis: A type of skin condition that often affects kids or young people. It causes little red spots, usually on the arms and torso. Things like a respiratory infection, strep throat, tonsillitis, stress, injuring the skin, and taking certain medications like antimalarial and betablockers can make it worse.
  • Erythrodermic psoriasis: A severe form of psoriasis that causes the skin to become bright red and shed scales in large amounts. Severe sunburn, infections, certain medications, or stopping certain psoriasis treatments typically trigger this condition. It is important to get treatment for it right away, as it can lead to serious illness.

What can Cause Psoriasis?

 

It is believed that an immune system problem causes psoriasis, though the exact cause is still uncertain. It is speculated that a combination of factors is to blame. Changes in the immune system cause inflammation, which sets off new skin cells to form at a faster rate. Normal skin cells grow every 10–30 days while in psoriasis, skin cells multiply every 3–4 days. This leads to the build-up of skin cells, causing silver scales.

 

Although psoriasis can’t be passed on from person to person but may be passed down the generations.

 

What are the Symptoms of Psoriasis?

 

One usually encounters a rash on the skin as the most common symptom of psoriasis; however, the symptoms may vary depending on the type of psoriasis. The symptoms can range from scaly, dry patches to puss-filled bumps; from joint inflammation to red spots all over the skin. Furthermore, one can also see pitting and discoloration of nails and in some cases, complete detachment of nails from the nail bed.

Severe psoriasis can lead to infections in the respiratory tract as well.

 

Diagnosis of Psoriasis

 

There are mainly two ways, a dermatologist will deploy to diagnose psoriasis.

  1. Physical Examination: The doctor checks for scaly patches (plaques) in areas such as the scalp, ears, nails, knees, belly bottom, and elbows.
  2. Lab tests: Your doctor will suggest you biopsy to check for psoriasis.

What are the Treatment Options for Psoriasis?

 

Fortunately, there are many cures. Some slow the growth of new skin cells and others relieve itching and dry skin. Your doctor will choose a treatment plan that is suitable for you based on the size and spot of your rash, your age, your general health, and other factors.

There are three types of treatments for psoriasis, depending on the severity of the condition. They are:

 

1. Topical treatments

 

These are the most common treatments for psoriasis, which include moisturizing lotions, steroid creams, and retinoid gels. Others include:

  • Anthralin: A topical cream that is used to control the growth rate of skin cells by reducing DNA synthesis
  • Calcineurin inhibitors: Immunosuppressants used in managing autoimmune conditions
  • Vitamin D-based cream or ointment
  • Coal tar: A typical remedy for scalp psoriasis available in lotions, creams, foams, shampoos, and bath products
  • Salicylic acid: Other topical treatments are generally combined with it, as it facilitates the scalp to take up the medication more easily.

 

2. Systematic treatments

 

When other treatments fail to produce results, systematic treatments are administered to the patient. These therapies are only given for a short duration so that you can get back to the milder treatment that you apply to your skin. Doctors give systematic treatments in the form of oral pills or injections.

  • Methotrexate: An immunosuppressant used to decrease the production of skin cells and suppresses inflammation; can’t be used for more than 6 months
  • Cyclosporine: An immunosuppression agent similar to methotrexate; can’t be used for more than a year
  • Biologics: Such drugs, usually injected, modify the immune system in a way that disrupts the disease cycle and improves symptoms and signs of disease within weeks
  • Steroids: Your healthcare provider may administer a shot of triamcinolone right into the persistent psoriasis patches.
  • Retinoids: Acitretin and other retinoids are drugs taken orally to reduce the production of skin cells.

 

3. Light Therapies

 

Light therapy is a prime treatment for moderate to extreme psoriasis, either alone or combined with medicines. It requires subjecting the skin to controlled amounts of natural or artificial light. Repeated treatments are necessary. These include:

 

  • Heliotherapy: Brief, daily exposures to sunlight
  • Excimer laser: Strong UVB light beamed on the affected skin to reduce scaling
  • UVB broadband: Administering controlled amounts of UVB broadband light from an artificial light source can be used to treat psoriasis that does not improve with topical treatments.
  • UVB narrowband: More effective than UVB broadband, can be administered just 2–3 times a week
  • Psoralen plus ultraviolet A (PUVA): UVA exposure is used after psolaren (light-sensitizing drug) is taken to make the skin more receptive to the treatment.
  • Goeckerman therapy: A treatment that combines the use of coal tar with light therapy. This makes the skin more responsive to UVB exposure.
